New tensioners failing?

I had one of my neoprene tensioner guides come off after about 5k. So I bought another tensioner and replaced.
For your tensioner replacement, did you drop the pan and clean out and into the oil pickup? I wonder if you had a guide piece in it and it came out and is batting around your pan now.
