Considering the fact that it goes on and off while driving (cycles through the different issues) I would guess that it's the module not the motor. No clue how to test that hypothesis without changing the module though. Would you know the part no.?

We don't know that the EPB goes on and off while driving, only that the warning does. Presuming that it's the module is jumping the gun.

If the car moves forward with the warning flashing, the EPB is stuck off and is not applying itself successfully when it's supposed too.
