Post Lots of Problems 1 in Particular

Attached is my scan from my Actron scan tool. I think it was actually frame 2 I started up my car. I know my check engine is on because I replaced the the coolant temp sensor, fixed for a bit, now have ordered a thermostat that is on the way. My main question is about a problem that has been going on for some time now. It has been way before the false reading of overheating.

My car will act like it is bucking. It seems more often than not it is when going up a hill but will happen on off ramps to get on interstate. It seems like alot of time it is around 30-35mph. I would really appreciate if I could get some expert advice if you might be able to find by my scan what is causing this problem. It is greatly appreciated
