Default PCV and Oil Filter Housing Leak

I had a check engine light and scanned codes P2187 and P2189. I got a PVC kit and replaced it. The old one seemed to have a slight tear in the diaphram but not completely sure because it was kind of stuck to the housing and I pulled it off so possibly it did when I did that. I assume with the codes it was torn. I noticed there was some oil residue on the oil filter housing but I hadn't changed the oil in a while so thought maybe was leftover or something. I looked a couple days later and there was some oil dripping down from the housing so I have a new filter for a change and used the new o-ring and replaced it. That day I checked again and no oil so I thought problem solved. I looked a couple days later and some oil had come out and pooled up below again. What could be causing it? Too much pressure and pcv isn't venting properly? The hose from the valve cover, past the filter housing, to the throttle body has a little bit of oil showing on the throttle body port. Any ideas what else to do or check?
