Default Poor Installation or Common Issue

Good evening. Recently I had the fuel lines replaced on my 1990 V12 XJS. The original owner never replaced the lines in 27 years.

For the second time in two weeks, gas is leaking from the fuel injector connections -- nothing that a couple of turns of a flat head can't fix.

My question is this due to poor installation or do these connections rattle loose from engine vibrations? I am hyper sensitive on preventing engine fires...

which connections exactly? You mention injectors, if so they should be push on using the cups/ ferrules and not clamps.
Another issue can be the use of incorrect size hose. They should not "rattle" loose and apparently didn't for 27 years.

There should not be any need for hoseclamps on the injector lines if the proper size hose was used.

I wonder if the old hose was cut off with a knife lengthwise? That's often the case and created a nick on the hosebarb from the blade and that's a common leak point.

The proper size hose required for these injector connections is not common in the U.S. and commonly available hose is a loose fit that requires extra compression to seal. Very wrong but looks fine while installing. I'd put money on your fuel lines being the wrong I.D. Correct lines won't leak when new even without the ferrules or any clamps.

My 1990 XJS(33K miles) hoses are original and still look fine...
