Have you inspected the clutch master and slave cylinders for leakage or a mechanical fault? Has the hydraulic fluid been changed and is it topped up correctly?

How many kilometres/miles on the vehicle?

It is also possible there is a problem developing with the clutch pressure plate diaphragm.
