4x4x4, how many times have you pulled the lever? Depending on how far back you pushed the piston, it may take a few cycles of the lever to get the brakes adjusted to where they need to be. I would say to run the handle up/down say 10 times and then see what you have. Otherwise, you may need to pull the wheels and manually operate the e-brake lever to get the pads close to where they need to be and then use the lever to make the final little bit of an adjustment.

4x4x4, from the sounds of things, then you have a snapped cable. I would say to first start with jacking up 1 side of the car and pulling on the handle. Then you can look under the car at the splitting beam that takes the single cable from the e-brake lever and splits it into the 2 cables for each wheel. The rear wheel that the lever is pointing to is the side with the issue. From there, you can remove the wheel and see if the e-brake lever is being moved. If it doesn't move, then you have a bad cable.

The only other thing that I can think of is you have a failed e-brake mechanism inside the caliper. You can prove this by manually operating the e-brake lever and one side you should feel it only be able to move about half way where the other side will travel to the stop. The side that goes to the stop is the bad side.

Hope this helps.
