Is the transmission fault warning lamp illuminated in the instrument cluster? If so, your transmission is in Limp Home Mode (LHM), which locks it in 3rd gear. On our '93, the most common cause of LHM was oil contamination of the electrical connector for the Throttle Position Sensor (TPS). This connector clips to a bracket on the intake manifold elbow just to the left of the throttle body. Separate the two halves and flush them with zero-residue contact cleaner (CRC is a well-known brand). Allow to dry and reconnect.
Regarding the clunk when you put it in Reverse, what is your engine rpm at idle?
Cheers,
Don

Your idle is too high - it should be closer to 650-700 rpm. You may have a vacuum leak or a problem with the Idle Air Control Valve (IACV). See the photo albums at the link in my signature for information about cleaning the IACV.

I don't have a picture of the transmission warning lamp, but your Driver's Handbook/Owner's Manual probably contains an illustration. Basically, the light looks like the outline of a transmission housing, possibly with an X through it, and if I recall correctly, it is on the left edge of the instrument cluster.
