Default fail safe engine mode no codes

2001 S type, 4.0 get the fail safe engine mode every time you touch the gas pedal, orange light only, no red light or limp mode. There are no codes, I even depressed the pedal causing the fault then read the codes, still no codes. Just replaced the water outlet pipe that the top hose hooks to, bled air and used proper anti-freeze. Thought I had maybe forgot to plug something back in but all looks good. This started after about 30 miles driving after changing the outlet pipe. Whats next? Thanks!

I think I posted about this quite recently:
use OBD to check the monitors are all Complete
check critical engine sensors are all plausible values (engine cold & engine hot, etc)
check engine goes CL (closed loop) on both banks soon after switching on and stays that way
check fuel trims
check all 4 O2 sensors (need to graph their waveforms) if you like but I don't suppose this step matters

I expect something will obviously be wrong fairly early on in the above.

Please post more car details such as mileage, whether gearbox oil changed / level checked, whether tensioners etc changed, other servicing done/needed (fuel filter, for example), and what if any work has been done recently.

Well, the problem ended up being a loose connector on the gas pedal. The car has 108400 miles, 2500 miles ago had a new engine due to tensioner failure and a new transmission due to gearbox failure. Thanks for your reply!
