Lurch and Hesitation in Transmission

Lurch and Hesitation in Transmission
Hi,
My S-Type, probably like many others, lurches forward when I switch from park to drive. As well as lurching, the car hesitates to kick into drive, and hesitates between reverse and drive (this can be annoying in a parking lot). Has anyone found a fix for either of these issues or a fix for both (as it sounds like they may be tied together)? Thanks.
