Default Driver Door Mechanism is Bust

First it went ka-chunk every time I pulled the door closed. Then it made a grinding sound. Now, the door opens wide but doesnt hold. It swings back to close immediately.

What is broken after 15 years and how do I fix it, and with what part?

sounds like something in the hinge assembly has broken. Should be easy enough to see how bad, looking in the door jamb when it's being held open. Without knowing exactly what is broken (and if you could take a picture of the hinges at all, that'd be a bonus), it's hard to say if you need the whole thing or just pieces.

You should have put some large washers on the studs when it started going "ka-chunk".

Now you might need to weld in some metal if it is torn out????? Might be body shop time.

Common problem on the sedans.

Guadin and Parts Geek have new check arms and shim that fits between the check arm and the door (necessary if the door frame has split, as Bob notes above). You need to pull the soft material away from the door jam and have a look; might also need to pull the door card to look inside to see if you need a new check arm or a shim or both.

I *** u me d that it was a torn door jam, bought the shim (in fact, large washers would have been sufficient), to find that the check arm had broken at one of the bolts. Easy fix once the parts are in hand, takes some patience. Search for broken check stop, door card removal, door repair -- and you will have enough research to take the job on. Careful with the plastic pieces that hold the door card to the door at the bottom, fragile and expensive from the dealer.

I had the same problem. Mine completely broke free from the steel door shell where the studs go through. I removed the door panel so I could see in from the back side. In my case the door has to come off to be properly welded by a body shop. It's in the shop as we speak...
