Use OBD to check IAT & ECT in case they're not working and thus spoiling fuelling.

Also check the fuel trims when it does run.

More and more sounds like a bad temperature sensor (it is in the crossover pipe, above the water pump). But first, disconnect the MAF sensor and check to see if you might have a bad or bent pen. Then, the primary fuel pump (how old is the fuel filter?).
